Q: Can submissions rely on public exchanges such as SHIN-NY?
Q: Many of the large urban academic medical centers have created patient portals that provide solutions to one or more of the five categories of challenges specified. Can submissions pull together "bits and pieces" of already-existing solutions?
Q: Are there online information sources that can tell me if a particular patient is currently covered by Medicaid, Medi-Cal, etc.?
Q: Can a submission use community-based organizations as designated intermediaries to this population?
Q: What is the cost impact of super-utilizers on the health care system? Can we quantify the monetary impact of one super-utilizer on the health care system?
